The instrumental (творительный паде́ж) answers with whom? (с кем), with what? (чем — the means, the tool) and is used for profession with the verbs рабо́тать (to work as), быть, стать (to become).

With whom? With what?
The preposition с (with) + instrumental expresses accompaniment. Without a preposition, the instrumental alone expresses the means (with which tool).
Accompaniment (с) and means (no preposition)
с + instrumental → accompaniment
Я иду́ в кино́ с дру́гом. (I’m going to the cinema with a friend)
Ко́фе с молоко́м и са́харом. (coffee with milk and sugar)
Instrumental alone → means, tool
Я пишу́ ру́чкой. (I write with a pen)
Мы еди́м суп ло́жкой. (we eat soup with a spoon)
Profession: рабо́тать / быть + instrumental
To state someone’s job with a verb, put the profession in the instrumental. Note: in the present with “is…” and no verb, you stay in the nominative (Он врач), but as soon as you use рабо́тать, быть, стать, it becomes the instrumental.
